Requirements to apply for proof of Canadian Citizenship for children born in Chile from a Canadian father or mother:
- Original Chilean Birth Certificate, issued by the Chilean Civil Registry;
- Original "Partida Integra de Nacimiento" (copy of the birth entry) issued and stamped by the Civil Registry;
- Original Parent's Marriage Certificate, if applicable;
- Original proof of Canadian citizenship for the father or mother (if born outside Canada: Certificate of Canadian Citizenship; if born in Canada: Canadian Birth Certificate). The father or mother must have been Canadian at the time of the child's birth;
- Two photographs according to the specifications indicated on the application form. You may select a photographer of your choice.
- Supplementary ID document of the child and of the parents (if application done by-mail, notarized copies are accepted).
- Translation of documents which are not in English or French (see instructions below).
- Fully paid fees.

Translations
Any document that is not in English or French must be accompanied by:
- the English or French translation, and
- an Affidavit from the person who completed the translation
Translations may be done by a professional translator whose certification can be confirmed, and must include an Affidavit. An affidavit is a document on which the translator declares in the presence of an Embassy Officer or at a Notary Public that the content of the translation is a true translation and representation of the content of the original document.
Translations can also be done at translation centres, and they must be accompanied by a certificate signed by the Director of the Centre, indicating that the translation was done by a professional translator and that the content of the translation is a true translation and representation of the original document.
If the translation is done at the Ministry of Foreign Affairs, Agustinas 1320, Santiago, an Affidavit is not requested.
Your application will be returned to you if you do not include the translation of your documents and an affidavit from the translator.
Chilean passport
Please note that if your child was born in Chile, they will have to leave the country on a Chilean passport. Please note that children are subject to local regulations applicable to the exit of minors from Chile. For further information, please contact local authorities.

Notarial services
Consular officials do not provide any legal advice or represent a person in court. The office provides the following notarial services, by appointment only, to Canadian citizens and, in limited cases, to non-Canadian citizens if the documents are intended for use in Canada.
Note: Documents must be in English or French, or accompanied by an official translation in English or French.
Authentication/Attestation (certify the document signature, the position of an official and the seal are genuine):
- Documents issued in Canada must be authenticated by Ƶ in Ottawa before the office will certify the documents (fees apply).
- Witness a signature on a Statement in lieu of certificate of non-impediment to marriage abroad form (fees apply).
- Clients requiring information about authenticating documents while outside of Canada, refer to Authentication of documents.
Certified copy (certify the document is a true copy of the original):
- Documents presented by a Canadian citizen or a non-Canadian citizen for use in Canada; exceptions apply (fees apply).
- Government-issued identification requiring a certified true copy (fees apply).
Witness a signature (consular officer witnesses a signature):
- Documents presented by a Canadian citizen or a non-Canadian citizen for use in Canada; exceptions apply (fees apply).
- Government-issued identification requiring a witnessed signature (fees apply).

We accept Visa and Master Card credit card payments. Please complete and sign the and attach it with your passport application. This form is mandatory in order to process your credit card payment if credit card holder is not coming to the Embassy.
Walk-in applications:
- payable in cash (exact amount) at the Embassy in Chilean Pesos or Canadian dollars. Canadian dollars in cash are accepted, provided exact amount is given. Please note we cannot accept coins or bills that are damaged or marked in any way.
- by credit card.
Mail-in applications:
- Money Order (“Vale Vista”) in the name of “Embajada de Canadá”. RUT number of the Embassy is required. Please contact the Consular Section of the Embassy to obtain this information.
